Why the impression collection slip matters
When a physical impression arrives at the lab, the slip is what ties it to the right case. A completed impression collection slip tells the team whose impressions these are, what you want made and which arch or notes go with them, so nothing sits unmatched on a bench waiting to be identified. Filling it in fully means your case is logged the moment it lands and routed straight to the right treatment planner, which is the simplest way to keep turnaround quick and avoid the back-and-forth that a missing detail causes.
What to include
- Doctor name, patient name and date
- Submission type, Active Aligners, Refinement, Smile Design, Retainers, Bite Plate or Sports Guard
- Whether it’s an overlay or clinical-feedback request
- Any special instructions or comments
